Stoma confidence before leaving home

Setting: A stoma nursing follow-up appointment

A patient can empty a pouch but needs prompts when replacing it. They are afraid of leakage outside the home. The prescribed pouch and skin-care plan are supplied.

Task

  • Explore the specific fear and its effect on activity.
  • Check which care step needs support.
  • Offer paced private practice without guaranteeing no leaks.
  • Agree supply follow-up and check understanding.
Stoma support and supply record
  • The patient can empty the pouch but needs prompts to measure the template and fit a replacement.
  • Offer private demonstration followed by observed practice; the physical pouch change follows this communication role-play under the existing specialist plan. No unsupplied procedure is tested here.
  • Do not promise that leakage can never occur.
  • Seven days of prescribed pouches are available at home. Registration with the supplier is submitted; delivery is unconfirmed. The stoma team will check delivery today and telephone the patient with the arrangement.
  • Partner teaching requires the patient’s agreement. The stoma team contact is on the supplied support card for equipment or care-plan questions.

Fictional scenario information supplied by the treating team; not medical guidance for real patients.
