Task

  • Explore the concern behind the opening: "The taxi company says they cannot take the chair. Everyone keeps telling me the car is booked."
  • Clarify the specific barrier and acknowledge its effect on the patient.
  • Explain the supplied next steps and respond to this possible question: "Could my son just lift me into his car instead so I can leave?"
  • Check the patient can distinguish what is agreed from what needs confirmation.

  • The nurse can tell the discharge coordinator about the reported mismatch and request confirmation of suitable transport.
  • The relevant team must confirm the planned mobility and assistance arrangements; the nurse should not improvise a transfer technique.
  • Do not promise a vehicle type, demonstrate unsupplied transfer steps or pressure the patient to attempt an unconfirmed journey.
  • I will tell the discharge coordinator what the company said and ask them to check the vehicle against your planned mobility arrangement.
  • The team needs to confirm the assistance and transfer arrangements as well as the vehicle. A car booking by itself does not show that the whole journey has been checked.
